Lakeside Family Dentistry Financial Guidelines
Mike Giebenhain DDS PA
We believe that financial considerations should not get in the way of the best care for you and your family and that everyone should have the opportunity to improve their dental health and appearance. This is why we accept most major insurance programs and offer several payment options.
Payment Options:
For your convenience we accept cash, check, money order, Visa, Discover, American Express, MasterCard, HSA accounts and Care Credit.
Estimated patient portion is due at time of service.
Dental Insurance:
As a courtesy to you we will bill your insurance directly. However, since dental insurance is a contract between you and your insurance company, your estimated portion is due at the time services are rendered and you are ultimately responsible for all charges incurred. Please remember that dental insurance is not meant to be a pay-all, but merely an assistance.
A savings adjustment is given to patients without dental insurance for payment in full with cash or check. We also offer a savings for seniors over the age of 60 who don’t have dental insurance, for payment in full with cash or check.
If you are unable to be at your scheduled appointment we ask that you contact our office 48 hours prior or you may be charged a fee.
Please feel free to ask any questions you may have regarding these guidelines.
